Uterine Artery Embolization

In which conditions do uterine artery embolization has a role:

  1. Post-partum haemorrhage (Bleeding after delivery or caesarean section) 
  2. Uterine Fibroid 
  3. Uterine Arteriovenoous malformation 
  4. Uterine adenomyosis 
  5. Management of placenta accrete and percreta 

Technique of uterine artery embolization

  • It is done under local anaesthesia through a groin artery (femoral artery) through a 2-3 mm incision. 
  • A catheter is advanced into abdominal aorta and angiogram obtained.
  • Uterine artery is selectively identified and catheterised. 
  • Subsequently embolization is done with an appropriate embolizing agent. 

Post-Partum haemorrhage(PPH) or Bleeding after delivery

  • Approximately 25-30% of such deaths.  
  • In India as per 2014 data, 190 women per lakh lost their life during child birth. 
  • 14 million women in developing countries experience PPH—26 women every minute. 
  • Traditional treatment for refractory PPH is Internal iliac artery ligation and hysterectomy(removal of uterus). 
  • Hysterectomy in young females may have many psychosocial, emotional and physical issues. 
  • No further pregnancies can sometime have detrimental effect on the family. 
  • Also it is associated with premature ovarian failure and hormonal imbalance.  

If timely performed, success rate of UAE for PPH is approximately 98%.

A study from our very own institute “PGIMER Chandigarh” of 50 patients showed technical success rate

  • Uterine artery embolization should be the treatment of choice & should be regarded as gold standard for severe PPH with the failure of conservative treatment. 
  • It should be performed before patient goes into shock or severe DIC develops. 
  • Menses, Fertility, Pregnancy all appear to return to normal post embolization.
